PRELIMINARY APPLICATION FORM

If your organization is eligible to apply (Maryland county, municipal, and 501(c)(3) organizations only): Download the Preliminary Application Form, complete, print and mail. Eight (8) copies of both the preliminary application, and the IRS Letter of Determination for 501 (c) (3) organizations must be submitted. No faxes. Handwritten applications not acceptable.
